Can fatty liver be reversed?
Liver fat can improve substantially in many people, particularly with sustained weight loss, improved nutrition and physical activity. Earlier disease is generally more amenable to improvement than advanced fibrosis or cirrhosis. NIDDK
What is MASLD?
MASLD stands for Metabolic Dysfunction-Associated Steatotic Liver Disease, the newer terminology used for what was previously commonly called NAFLD when liver steatosis occurs in the appropriate metabolic context. NIDDK
Can liver enzymes be normal with fatty liver?
Yes. Liver enzymes alone cannot reliably exclude steatotic liver disease or more significant pathology. Imaging and fibrosis-risk assessment may therefore be important depending on the clinical context.
Does insulin resistance cause fatty liver?
Insulin resistance is strongly associated with MASLD and contributes to the metabolic environment in which excess liver fat develops, but the disease is multifactorial.
Does fatty liver cause high triglycerides?
Fatty liver and elevated triglycerides frequently coexist as part of broader insulin resistance and metabolic dysfunction. Neither should automatically be considered the sole cause of the other.
Should I stop fruit if I have fatty liver?
Not automatically. Whole fruit can form part of a healthy diet. Fruit juices, sugar-sweetened beverages and excessive concentrated sugar intake are different issues.
Is fructose bad for fatty liver?
High intake of fructose-containing added sugars, particularly from sweetened beverages and processed foods, has been associated with fatty liver risk. This should not be interpreted as meaning all whole fruit needs to be eliminated. NIDDK
Is keto good for fatty liver?
A lower-carbohydrate approach can be useful for selected people, but keto is not mandatory. The overall calorie intake, fat quality, fibre, metabolic response and sustainability of the diet matter.
Should I stop seed oils for fatty liver?
In my practice, I commonly reduce seed-oil-heavy fried and ultra-processed foods as part of improving dietary quality. However, current clinical guidelines do not establish avoidance of all seed oils as a specific MASLD therapy. Evidence is stronger for reducing excess saturated fat and improving overall dietary quality.
Is milk thistle useful for fatty liver?
Milk thistle has been studied in liver disease, but evidence is not strong enough to replace established interventions such as weight management, appropriate nutrition and exercise.
Can NAC help fatty liver?
NAC has biological roles related to glutathione metabolism and has been investigated in liver health, but it should be considered an adjunct rather than established primary therapy for MASLD.
Should alcohol be stopped?
In my practice, I generally remove alcohol when someone has confirmed fatty liver. Individual medical recommendations should also consider the type and severity of liver disease.
Is ultrasound enough to monitor fatty liver?
Ultrasound can identify steatosis but has limitations. Depending on the person’s risk, fibrosis assessment such as FIB-4, elastography/FibroScan or specialist evaluation may be necessary.
Can I consult online for fatty liver?
Yes, where remote functional-medicine support is appropriate and relevant medical/hepatology care continues when necessary.
