Brain health is influenced by far more than the brain alone.

Sleep, nutrition, gastrointestinal health, nutrient status, inflammation, immune activity, physical activity, stress, medications and the autonomic nervous system can all influence how we think, feel, sleep and function.

At RH+, our approach to brain and neurological health therefore begins by understanding the wider physiological environment in which neurological or cognitive symptoms are occurring.

With Prashantt J Wadhaawan, Functional Medicine Practitioner and Founder of RH+, the objective is not to replace neurological or psychiatric care. It is to identify potentially modifiable factors that may be contributing to the individual’s overall symptom burden and health.

Brain Symptoms Do Not Always Begin in the Brain

People often think of neurological symptoms as isolated problems.

Poor memory means a brain problem.

Anxiety means a psychological problem.

Migraine means a headache problem.

Brain fog means poor concentration.

But human physiology rarely operates in isolated compartments.

The brain continuously receives information from the gastrointestinal system, immune system, endocrine system, autonomic nervous system and metabolic environment.

That does not mean every neurological condition originates outside the brain.

It means that when someone experiences persistent brain, mood or neurological symptoms, it can be useful to examine the broader physiological context rather than looking at one symptom alone.


Brain & Neurological Concerns I Have Encountered in Practice

Over the years, people have come to me with a wide range of brain, behavioural, cognitive and neurological concerns.

These have included:

  • Brain fog
  • Poor concentration
  • Memory difficulties
  • Migraine and recurrent headaches
  • Anxiety

  • Sleep problems

  • Autism-spectrum conditions

These conditions are not equivalent and should not be approached as though they share one underlying cause.

Conditions such as ADHD, autism, bipolar disorder, addiction and significant psychiatric disorders may require specialist medical, psychiatric, psychological or neurological care.

At RH+, our role is to identify whether nutrition, gut health, nutrient deficiencies, sleep, inflammation, lifestyle or other modifiable factors may be adding to the individual’s overall health burden.


How I Assess Brain & Neurological Symptoms

When someone presents with brain fog, poor concentration, memory difficulties, migraine, anxiety or sleep disturbance, I do not automatically begin with a neurological supplement protocol.

I first try to understand what else is happening in the person’s biology.

Depending on the individual, I pay particular attention to:

  • gastrointestinal health
  • food allergies
  • food sensitivities and reproducible food-related symptoms
  • possible histamine-related reactions
  • nutritional deficiencies
  • dietary patterns
  • relevant infectious history
  • inflammatory patterns
  • sleep
  • exercise and physical activity
  • recovery
  • autonomic nervous system state
  • medications
  • previous antibiotic exposure
  • prolonged or recurrent use of acid-suppressing medication
  • medical and psychiatric history

The purpose is not to prove that one of these factors caused the neurological problem.

The question is:

Is something outside the brain adding to the burden being experienced by the brain?


The Gut–Brain Axis Is Central to Our Assessment

The gut–brain axis is one of the most important areas I consider when assessing brain, mood and cognitive concerns.

The brain and gastrointestinal system communicate bidirectionally through several pathways involving the nervous system, immune signalling, microbial metabolites, endocrine signalling and other physiological mechanisms.

Modern research increasingly recognises the microbiota–gut–brain axis as an important area of brain-health research, although many of the mechanisms and therapeutic implications are still being investigated. Nature

In practice, I frequently see people with brain or mood complaints who also have some combination of:

  • bloatingGut & Neurological Health, Gut Brain Axis
  • altered bowel habits
  • reflux
  • food-related symptoms
  • nutritional insufficiencies
  • restrictive diets
  • repeated antibiotic use
  • digestive problems they have considered “normal”
  • previous gastrointestinal illness

This does not mean that everyone with neurological symptoms has a gut disorder.

Nor does it mean that treating the gut will treat a neurological or psychiatric diagnosis.

But I consider the gut–brain relationship important enough that gastrointestinal health deserves routine assessment rather than being ignored.

Food Allergies, Food Sensitivities & Histamine-Related Symptoms

Food-related reactions can occur through different mechanisms.

A true food allergy involves an immune response and may trigger mast-cell mediators including histamine.

Other food-related symptoms may involve intolerance or different mechanisms and should not automatically be labelled as allergy.

In susceptible individuals, histamine-related responses may be associated with symptoms extending beyond digestion, including:

  • flushing
  • headache
  • palpitations
  • sleep disturbance
  • anxiety-like sensations
  • changes in concentration or mental clarity

For this reason, when a person reports reproducible symptoms after particular foods, I consider that history as part of the broader assessment.

The goal is not to place everybody on a restrictive elimination diet.

It is to determine whether the food-related reaction is real, reproducible and relevant enough to warrant further evaluation.


Nutritional Deficiencies & Brain Function

The nervous system has significant nutritional requirements.

Depending on the individual, deficiencies or inadequate intake involving nutrients such as:

  • Vitamin B12
  • Folate
  • Iron
  • Vitamin D
  • Thiamine
  • Magnesium
  • Other vitamins and minerals

may contribute to neurological, cognitive or fatigue-related symptoms.

For example, vitamin B12 deficiency can produce neurological abnormalities, sometimes even before severe anaemia becomes obvious.

This is why nutritional status can be an important part of the assessment rather than assuming that every problem with memory, concentration or sensation originates from a primary brain disorder.

Testing should be based on the person’s history and clinical context.


Brain Fog & Poor Concentration

“Brain fog” is not a single medical diagnosis.

People use the term to describe experiences such as:

  • difficulty concentrating
  • reduced mental clarity
  • forgetfulness
  • slower thinking
  • difficulty finding words
  • reduced productivity
  • mental fatigue

Many different factors can contribute to these symptoms.

Depending on the individual, these may include:

poor sleep, nutritional deficiencies, inadequate nutrition, medications, chronic stress, gastrointestinal problems, inflammatory illness, thyroid dysfunction or other medical and neurological conditions.

Therefore, the question should not simply be:

“Which supplement is best for brain fog?”

A better question is:

“Why is this particular person experiencing reduced cognitive function?”


Migraine & Recurrent Headaches: Brain or Neurological

Migraine is a neurological disorder and should not be reduced to a gut problem, food sensitivity or nutrient deficiency.

However, individual migraine patterns may be influenced by multiple triggers and health factors.

These may include:

  • sleep disruption
  • missed meals
  • dehydration
  • stress
  • hormonal changes
  • certain foods in susceptible individuals
  • alcohol
  • medications
  • nutritional factors
  • other individual triggers

Migraine is also one of the neurological conditions contributing substantially to global neurological disability. World Health Organization

At RH+, the role is to identify modifiable factors surrounding the person’s migraine pattern while appropriate medical evaluation and treatment continue where required.


Effect of Chronic Infections, Antibiotics & the Gut Environment on Brain Health 

I do not usually assume that a neurological or behavioural symptom appearing after an infection means that infection directly caused the condition.

What interests me more is the long-term health timeline.

Some individuals have experienced:

  • recurrent infections
  • repeated antibiotic courses
  • prolonged gastrointestinal disturbance
  • long-term acid-suppressing medication use
  • progressive dietary restriction
  • nutritional problems

Antibiotics can alter the composition and diversity of the intestinal microbiome, sometimes for weeks or months after treatment, although the magnitude and duration vary greatly between antibiotics and individuals. PubMed

Proton-pump inhibitors and other forms of sustained acid suppression have also been associated with measurable alterations in gastrointestinal microbial composition. PubMed

This does not establish that antibiotics or acid-suppressing medication caused someone’s neurological condition.

But a history of repeated exposure may provide useful context when evaluating digestive health, nutrition and overall symptom burden.

Medication should never be stopped simply because of these associations. Any medication change should be discussed with the prescribing medical professional.


The Autonomic Nervous System & Brain Health: A Functional Medicine Perspective

Another area I pay close attention to is the autonomic nervous system — ANS.

Some people live for prolonged periods with signs of physiological hyperarousal.

They may report combinations of:

  • shallow or rapid breathing
  • palpitations
  • inability to relax
  • anxiety-like sensations
  • poor sleep
  • excessive vigilance
  • poor recovery
  • fatigue despite feeling “wired”
  • digestive changes
  • reduced tolerance to stress

These symptoms can have multiple causes and should be medically evaluated when appropriate.

But when an individual appears to spend significant time in a heightened sympathetic state, I consider factors such as:

sleep, breathing patterns, physical activity, stress exposure, recovery and daily routine.

The objective is not to label every problem as “nervous-system dysregulation.”

It is to determine whether autonomic state is one meaningful part of the individual’s wider health picture.


Sleep & Brain Health

Sleep is one of the fundamentals I rarely overlook.

Poor sleep can affect:

  • concentration
  • memory
  • mood
  • emotional regulation
  • reaction time
  • appetite
  • physical recovery
  • pain perception
  • overall daytime function

Before looking for increasingly complicated explanations for cognitive symptoms, it is essential to understand:

  • sleep duration
  • sleep timing
  • sleep quality
  • night awakenings
  • snoring or possible sleep apnoea
  • shift work
  • stimulant use
  • alcohol
  • screen and light exposure
  • stress

Sometimes the most sophisticated intervention begins with correcting something basic.


Exercise, Movement & Brain Health

Physical activity is not only important for muscles and cardiovascular health.

Movement and regular exercise also form part of healthy brain ageing and overall neurological health.

The appropriate programme depends on the individual, but may involve:

  • resistance training
  • cardiovascular exercise
  • regular walking
  • mobility
  • coordination
  • reducing prolonged sedentary behaviour

This area is particularly important to the RH+ philosophy because physical capacity, muscle and metabolic health should remain part of brain-health conversations rather than being treated as separate subjects.


Inflammation & Its Relationship With Brain Health

Inflammation is an important part of immune defence and is not inherently harmful.

However, inflammatory illnesses and chronic inflammatory states can sometimes influence fatigue, cognition, sleep and general wellbeing.

At RH+, we do not simply label every neurological symptom as “neuroinflammation.”

Instead, we look at the person’s medical history, symptoms and relevant investigations to determine whether there is actually evidence that inflammatory or immune factors deserve attention.

Brain Health & Thyroid Function

Thyroid dysfunction can sometimes produce symptoms that overlap with neurological complaints, including:

  • fatigue
  • reduced concentration
  • cognitive slowing
  • mood changes

For this reason, thyroid history and relevant thyroid markers may deserve consideration when evaluating persistent cognitive symptoms.

Brain Health & Metabolic Health

Brain and metabolic health are also interconnected.

Conditions such as diabetes can cause neurological complications including diabetic neuropathy. WHO reported that diabetic neuropathy was the fastest-growing neurological condition globally between 1990 and 2021, paralleling the worldwide growth of diabetes. World Health Organization

This is one reason long-term brain health should not be separated completely from:

  • blood pressure
  • metabolic health
  • diabetes risk
  • physical activity
  • body composition
  • smoking
  • nutrition

ADHD, Autism & Functional Medicine

This section needs particularly careful positioning.

ADHD and autism are neurodevelopmental conditions.

Functional medicine should not be presented as a cure for either condition.

Where an individual with ADHD or autism also has problems involving:

  • nutrition
  • restrictive eating
  • gastrointestinal symptoms
  • nutritional deficiencies
  • food allergy
  • sleep
  • physical activity
  • medication-related issues
  • other health concerns

these areas may deserve appropriate assessment and support.

But improving one of these associated health problems should not be interpreted as “treating the autism” or “reversing ADHD.”

Appropriate paediatric, psychiatric, psychological or neurological care should continue where required.


Anxiety, Phobias, Mood Disorders in Functional Medicine

Anxiety, phobias and mood disorders are not simply nutritional or gut-health problems.

They can involve complex psychological, neurological, genetic, environmental and social factors.

However, physical health can influence how a person feels and functions.

Depending on the individual, areas such as:

  • sleep
  • nutritional status
  • gastrointestinal symptoms
  • food intake
  • stimulant or alcohol use
  • exercise
  • medication effects
  • physical illness

may still deserve attention.

For significant anxiety disorders, bipolar disorder, addiction or other psychiatric conditions, RH+ should work alongside appropriate mental-health care, not instead of it.


When a Neurologist or Psychiatrist Comes First

Functional medicine is not the appropriate first step for every brain or neurological concern.

Urgent or specialist assessment should take priority for symptoms such as:

  • sudden weakness or numbness
  • difficulty speaking
  • suspected stroke
  • new seizures
  • loss of consciousness
  • rapidly worsening memory
  • sudden major behavioural change
  • severe or unusual new headache
  • progressive neurological decline
  • psychosis
  • manic episodes
  • schizophrenia-spectrum symptoms
  • suicidal thoughts
  • severe psychiatric distress

Can gut health affect brain health?

The gut and brain communicate through neural, immune, endocrine and metabolic pathways, and the gut microbiome is actively studied as part of this relationship. However, the clinical significance differs between individuals, and gut treatment should not be presented as a universal treatment for neurological disease. Nature

Can gut problems cause anxiety?

The gut–brain axis can influence physiological processes relevant to mood and stress responses, but anxiety disorders are multifactorial. Gastrointestinal health may be one contributing consideration in some individuals rather than a universal cause.

Can food sensitivities cause brain fog?

Some individuals report reproducible cognitive or other symptoms after certain foods, but “food sensitivity” includes several different mechanisms. Appropriate assessment should distinguish true allergy, intolerance and other reactions rather than assuming every brain-fog complaint is food related.

Can histamine cause anxiety and palpitations?

Histamine and other mediators released during allergic reactions can produce systemic symptoms, including cardiovascular and neurological sensations. Symptoms such as recurrent palpitations or significant anxiety-like episodes still warrant appropriate medical assessment rather than automatically being attributed to histamine.

Can antibiotics affect the gut microbiome?

Yes. Antibiotic exposure can alter gut microbial composition and diversity, though the magnitude and duration vary according to the antibiotic and individual. Most microbiomes show substantial recovery, while some changes can persist longer. PubMed

Can functional medicine treat ADHD or autism?

Can functional medicine help migraine?

When should I see a neurologist instead?
